Bids for furnishing all labor, materials, equipment, and services required for the Work known as Bonifay FL 15-5 & FL 15-16 HVAC as directed by the PHA @ four (4) site’s known as FL 15-5 & FL 15-16 the installation of new heat pumps in fifty-three (53) dwelling units as well as providing new electrical for a new cook range at fifty-six (56) dwelling Units and all associated work will be received until 11:00 AM local time on 5 September 2019 at the office of the Housing Authority (PHA) indicated below. Without force and effect on the Bidding Documents and the proposed Contract Documents, the work required is briefly described as: The Work known as, removing existing gas fired air handlers and condensing units, all gas lines beginning at the existing gas meter and all gas lines throughout the unit, provide new electric heat pump and condensing units at fifty-three Units ( Units 726, 737 and 304 do not receive new heat pumps) provide new electrical service at all fifty-six units for a new electric cook range and new electrical as needed for the new heat pumps as well as removing all existing gas lines and all associated work, see outline of work and project manual for specific work items.

The work required is fully described in the Bidding Documents consisting of the Project Manual and the Drawings.

Proposed Contract forms, Drawings and Project Manual are on file in the office of the consultant Mr. Randall O’Barr, Post Office Box 357, Baldwin, Georgia 30511, telephone (706) 244-2911, and Fax (706) 754-4121. Bidding Documents may be obtained by providing a NONREFUNDABLE payment of $35.00 per set of Documents to the Consultant. No partial sets will be issued. Information regarding this Project, including a list of the Plan Holders will be provided upon request.

Each bid shall include Bid Guarantee in an amount equal to five percent of the Bid. Provide as a certified check or bank draft payable to the PHA; U.S. Government Bonds, or as a properly executed Bid Bond with surety acceptable to the PHA. A Surety Company executing the Bid Bond must be authorized to transact business in the Project State, and must appear on the most current U.S. Treasury Department’s Circular No. 570. The successful bidder is required to provide satisfactory Performance and Payment Bonds prior to execution of the Agreement.

Refer to provisions for equal employment opportunities and payment of not less than minimum salaries and wages indicated in the Project Manual.

Each bid shall include THE SIGNED ORIGINAL AND TWO CONFORMED COPIES of the following:

1. A properly executed Bid Form.
2. A properly executed Bid Guarantee.
3. A properly executed Non-Collusive Affidavit.
4. A fully completed Form HUD-5369-A, “Representations, Certifications and Other Statements of Bidders”.

Small businesses, section three business and minority firms are urged to submit proposals. Certification as a Minority-business Enterprise (or number of partners, shareholders, employees who are members of minority classification or are women) should be included in the Bid proposal. Refer to Articles 38, 39 and 40 of The General Conditions.

The PHA reserves the right to reject any and all bids, and to waive irregularities and formalities in the bidding. No bids may be withdrawn for a period of sixty days subsequent to the opening of bids without PHA consent.

The Northwest Florida Regional Housing Authority (NWFRHA) is currently seeking proposals for services from a consultant on a work for hire basis to provide Contract Administration in our Public Housing Development’s.

The following will be required:

1. Complete set of Construction Documents
   1. Architectural drawings
   2. Specifications of acceptable construction methods
   3. Specifications of acceptable materials and/or products
   4. Product and product data review for compliance
2. Construction Contract Documents
   1. Bid Forms
   2. Agreement Forms
   3. All applicable HUD forms
   4. Review of all forms for compliance
   5. Presence during preconstruction conference, if necessary
   6. Presence during bid opening and contract award
3. Observation during Construction
   1. Periodic site visits
   2. Written progress reports
   3. Pay request review and/or approval
   4. Preparation and review of all required close-out documents

Each proposal must include verifiable prior HUD experience.

The Housing Authority will accept an option for additional quantities of work or performance periods deemed applicable to the phases of our ongoing modernization program.

Proposals should include qualifications, related experience and proposed costs.
